Records Needed for a Second Opinion

Medical Records Needed for the Heart Second Opinion Program

  • Comprehensive medical history

  • Operative notes from any cardiac procedures performed

  • List of medications

  • CT scans for patients with aortic problems

  • Recent tapes and reports from diagnostic tests such as echocardiograms and cardiac catheterization

The tapes are important for our physicians to interpret your tests rather than relying just on the paper report. We may request other records depending on your specific diagnosis and what other testing or procedures you have had. 
 

How to Obtain Your Medical Records

You can obtain your medical records yourself or with our help. Sometimes patients can get the records faster directly through their referring physician's office. Please note some medical offices charge a fee to provide you with your medical records.

You can start the process by calling your doctor's office to get a copy of your medical history and test reports. You also will need to get actual tapes from recent diagnostic tests you have had such as echocardiograms and cardiac catheterization.
