Neurology
Most, but not all, neurology patients must be referred by their primary care physician.

Before your appointment, you will receive a packet of information in the mail. This will include a map of the hospital, our health history form and a letter of instructions.
The letter will include helpful phone numbers in case you have questions. Please complete the health history form and bring it, along with all information and X-rays that you have, with you to your appointment. Please also ask your physician to fax any relevant history and patient records prior to you appointment. The fax number for adult neurosurgery is (314) 747-3765.
Neuroradiology
Our neuroradiologists and interventional neuroradiologists perform thousands of examinations and procedures each year on an outpatient and inpatient basis. Some of these tests and procedures require advance scheduling while others do not. Most require advance authorization from the patient’s referring physician.
These different radiological examinations have their own special pre-exam requirements that sometimes include restricted patient diets or pre-medications. When you call for an appointment, you will receive information about the kind of preparation that your examination requires. You will also be told exactly where to come for your appointment.

When you arrive for your appointment, you will also need to fill out a patient questionnaire, which asks you to summarize your general medical, family and social history. It will become part of your permanent patient record.
